Who is eligible?
A person who has a terminal illness with a life expectancy of six months or less, as determined by their doctor.
Who pays for hospice?
Medicare
Medi-CAL
Health Insurance Plans
There is never a co-pay for Medicare or MediCAL
How do I start the admission process?
To be admitted to the Buena Vista Hospice Care program, call the hospice office (805) 777 -1133, and a nurse will be scheduled to visit the patient and family at home or in the hospital. Information about the program will be presented and if the patient elects the program, the nurse will arrange for the start of services.

What is Hospice?
Hospice is a program of health care and services designed to meet the special needs of patients and their loved ones at the end of life.

Hospice care can dramatically and positively impact quality of life. The goal is to empower the individual by providing knowledge and information to facilitate decision-making. Our clinical staff members are experts in pain control, symptom management and emotional counseling. They are committed to the education of the patients and caregivers.

How is Hospice Care Started?
Help is one phone call away. Any person concerned with a patient's well being can initiate a referral for hospice care. Usually the referral comes through a physician, discharge planner, social worker, or family member. Admission is a simple process.
Services Available
Caring for People in Times of Transition
Buena Vista Hospice Care offers a wide variety of services to support the patient and his/her family.
Medical Director, who works in conjunction with the patient's own physician and makes home visits as needed.
Skilled Nurses, who are trained specifically to work with pain and symptom management. A nurse is available 24 hours per day.
Social Workers, who are specially trained in grief, bereavement, and end-of-life issues. They can also offer information and connect patient and family with community resources.
Spiritual Counselors, who provide spiritual support and/or work as a liaison to the patient/family's clergy of choice.
Volunteers, who provide assistance and loving care through visits, letter-writing, reading, shopping, friendship, pet care, plant watering, and more.
Personal-care/homemaker/CHHA Homemaking services whose role is designed to provide peace of mind and comfort.
Medications & Medical Equipment, which are provided along with other supplies related to the terminal illness diagnosis at no additional cost to the patient or family.
A Wide Variety of Stress Management Techniques, Music Therapy, Massage Therapy, Guided Imagery, Aromatherapy, Reflexology, and Reiki.
Bereavement Program, ongoing grief counseling for friends and family.
